Case Management Services

Our TBI Case Management service is one-on-one, and assists the TBI survivor in locating medical care, residential services, legal services, substance abuse treatment, education and benefits assistance. Additionally, the Case Manager assists in arranging needed tests, community referrals, and transportation needs.

We provide a needs-assessment, identify problems and design appropriate interventions to address any problems. We also arrange requisite support services (such as housing, benefit programs, job training, neuropsychological evaluations), monitor medication and use of services, and advocate for client rights and entitlements.

People wearing suits ready to help This service is not a time-limited service, but is intended to be ongoing, providing clients whatever they need whenever they need it, for as long as necessary. This insures the client attains the highest quality of care and is intended to address the TBI client’s biopsychosocial needs.
